The difference is the machine, the comfort, the treatment time, and the price.

An honest, unbiased view

I do not offer Ultherapy, and I want to be transparent about that, but I also want to give you an honest clinical picture of both, because clients ask me about Ultherapy constantly.

Many clients come to me having seen Ultherapy advertised at significantly higher prices and wondering whether it is worth paying more. This guide is my honest answer. What both treatments actually do

HIFU stands for High Intensity Focused Ultrasound. Both Ultherapy and the Ultraformer III use this same core technology to deliver focused ultrasound energy to specific depths beneath the skin, including the SMAS layer (Superficial Musculoaponeurotic System).

The SMAS layer is the same structural layer targeted during a surgical facelift. By heating this layer with ultrasound energy, both devices trigger your body to contract existing collagen and produce new collagen over the following months, resulting in gradual lifting, tightening and improved facial definition.

What is Ultherapy specifically?

Ultherapy is the brand name for a HIFU treatment using the Ulthera device, manufactured by Merz Aesthetics in the United States. It was the first HIFU device to receive FDA clearance for non-invasive brow lifting, and it has built significant brand recognition off the back of that first-mover advantage.

Its standout feature is MFU-V technology, Microfocused Ultrasound with Visualisation. This allows the practitioner to see a real-time ultrasound image of the skin layers during treatment, which theoretically enables more precise targeting of the SMAS layer.

Ultherapy delivers energy in a linear sequence using a single transducer, which means treatment sessions typically take 1.5 to 2 times longer than comparable Ultraformer III treatments.

In terms of comfort, Ultherapy is generally considered more intense. The higher energy levels required by its single-transducer approach mean most clients experience more discomfort, and topical anaesthetic is often recommended.

What is the Classys Ultraformer III?

The Ultraformer III is a HIFU device developed by Classys, a South Korean medical technology company. It is a medical-grade device used in clinical research settings and is one of the most widely used HIFU systems globally.

Unlike Ultherapy's single-transducer approach, the Ultraformer III uses multiple interchangeable cartridges that can target multiple depths within the same treatment session: from 1.5mm (superficial dermis) through 3mm (deep dermis) to 4.5mm (SMAS layer) and beyond. This versatility allows a single treatment session to address multiple skin concerns at different depths, tailored to the individual client's anatomy.

The Ultraformer III's multi-depth approach also means it delivers energy more efficiently. Treatment times are typically 30 to 50% shorter than Ultherapy for the same area, and the treatment is generally considered more comfortable due to the more controlled, precise energy delivery.

The real-time imaging question: does it actually matter?

This is Ultherapy's most significant technical differentiator. The ability to visualise skin layers in real-time during treatment sounds compelling, and in theory it allows the practitioner to see exactly where they are delivering energy.

The honest clinical picture is more nuanced.

Real-time imaging is genuinely useful for ensuring energy is delivered to the right depth, particularly around areas where the SMAS layer varies in depth between individuals. This is a real technical advantage.

However, the clinical outcomes between Ultherapy and well-calibrated HIFU devices like the Ultraformer III are broadly comparable in published research. The Ultraformer III compensates for the absence of live imaging through its multiple cartridge depths and precise calibration, allowing experienced practitioners to deliver energy accurately without real-time visualisation.

In my clinical experience, the results I achieve with the Ultraformer III are at least equal to the Ultherapy results my clients have shown me from other clinics, and frequently more comfortable for the client during treatment.

Why does Ultherapy cost so much more?

This is the question clients ask most often when they compare prices, and it deserves a direct answer.

Ultherapy's premium pricing comes from several factors:

Brand recognition. Ultherapy was the first FDA-cleared HIFU device. It has spent over a decade building brand awareness through marketing, celebrity endorsements and high-profile clinic partnerships. That brand recognition commands a premium.

Device cost. The Ulthera machine costs significantly more than the Ultraformer III, and clinics pass that cost to clients.

Marketing positioning. Many clinics position Ultherapy as a premium tier product and price it accordingly, sometimes charging more because clients expect to pay more for a branded treatment.

The real-time imaging feature. The MFU-V technology adds genuine value and cost.

None of this means Ultherapy is not a good treatment. It is. But the price difference does not reflect a proportional difference in clinical outcome for most clients. You are paying partly for the brand and partly for the imaging technology. Whether that premium is worth it is a personal decision.

My honest clinical verdict

I use the Classys Ultraformer III. I believe it is an outstanding device that delivers consistent, clinically meaningful results for my clients. I am being transparent that I have a commercial interest in saying this, but I am also confident saying it because I see the results in clinic every week.

In my experience, clients who have had Ultherapy elsewhere and then switch to HIFU with the Ultraformer III at my clinic report results that are comparable or better, at a significantly lower price point, and consistently describe the treatment as more comfortable.

The clients for whom I might still suggest exploring Ultherapy are those who specifically want the real-time imaging reassurance, or those who are being treated in a medical setting where visualisation of tissue depth is clinically indicated for their specific anatomy.

For the majority of clients seeking facial lifting, jawline definition, neck tightening and collagen rebuilding, the Ultraformer III delivers the same mechanism, the same tissue target, and in my experience a very similar lifting outcome for suitable clients, at a fraction of the price.

A real client scenario

A client came to me in her late 40s. She had researched Ultherapy extensively, had seen it recommended by influencers, and had received a quote of £2,400 for a full face and neck treatment at a central London clinic.

She came to me for a second opinion before committing.

After assessing her skin and discussing her concerns, mainly jawline softening and some neck laxity, I explained that the Ultraformer III targets the same tissue layer as Ultherapy using the same fundamental mechanism, and that the result she wanted was achievable at my Fulham clinic at a significantly lower price.

HIFU with the Ultraformer III is an excellent choice if:

  • You want to address neck laxity without surgery
  • You want a treatment with minimal downtime
  • You have been quoted for Ultherapy and want to understand whether a comparable result is achievable at a lower price
  • You want a treatment that can target multiple tissue depths in a single session
  • You are in Chelsea, Battersea, Putney, Knightsbridge or anywhere in South West London and want a clinic you can trust

Who might Ultherapy be more appropriate for?

  • Clients in a medical setting where real-time tissue visualisation is clinically indicated
  • Clients who specifically want the reassurance of FDA-cleared branded treatment regardless of price
  • Clients whose anatomy presents particular depth variation that benefits from live imaging guidance

If someone specifically wants real-time ultrasound visualisation, is comfortable with the higher price, and is being treated in a medical setting where imaging guidance is important, Ultherapy may well be worth considering. My point is not that Ultherapy is bad. It is a genuinely good treatment. My point is that many clients do not need to pay Ultherapy prices to achieve meaningful lifting results.

Practical considerations for London clients

HIFU with the Ultraformer III is a lunchtime treatment. You can come to my Fulham clinic, have the treatment, and head straight back to Chelsea, Knightsbridge or Putney with no visible signs. Some warmth and mild tenderness during treatment. Occasional mild transient swelling that resolves within hours.

Ultherapy is similarly low in visible downtime but typically involves a longer treatment session and often more discomfort during the procedure itself.

Neither treatment is appropriate if you want instant results. The collagen rebuilding process takes time, and results develop gradually over 2 to 6 months.
